Our hospital to home service provides you with personnel that enables you to return home after a stay in hospital whatever level of care you require. During your recuperation we are able to provide a care team that can accommodate any of your requirements whether that be a registered nurse, health care assistant or care worker.

Our hospital to home service helps you if you require assistance :

  •     On discharge from hospital to return to your home
  •     To return home but have decided that you require support to assist in your recuperation
  •     To plan post operative care prior to being admitted to hospital
  •     By providing transport and assistance with equipment that may be needed in your home to assist in your care

Heaven’s Colony is able to provide a flexible live-in care personnel (24 hour care service) across the UK allowing you the very best lifestyle choices in your own home.

Whether it is making decisions with regard to your daily meals, undertaking your favourite pastimes or watching your favourite TV programmes on television, our live in care service will ensure that you have full support to take care of your specific home care requirements.

Heaven’s Colony staff are well trained in palliative care provision. We provide support for individuals (adults or children) across the UK who have a life-threatening illness and who would like to remain living in the comfort of their own home surrounded by their family and friends.

Our palliative care service looks to provide the very best quality of life for you and your family, allowing you to share and discuss you feelings and personal needs at this difficult time.

Our palliative services enable you to receive support throughout your illness and can include :

  • Support after treatments
  • Ensuring you are comfortable at all times
  • Allowing you to remain as active as possible
  • Supplying personal care
  • Providing social companionship
  • 24 hr care or calls as and when required dependent on your illness requirements.

Caring for another person can be physically and mentally demanding, and much more than purely handling the day-to-day tasks. Especially when caring for long periods at a time, the emotional strain can take its toll.

Respite care provides your main carer with a temporary break while ensuring your care needs are met. The types of respite care available will ensure that those receiving care will have their requirements met by our expert carers and management team, whilst you look after yourself.

We have experienced care teams across the UK who provide support and care for those with physical disabilities including nurses, health workers and care workers.

When your loved one needs a little more help with daily routines, we provide a range of quality nursing and personal care services.

  • Personal care – assistance with getting dressed, self-care and toileting
  • Medical administration – help with taking medication and gentle reminders
  • Mobility support – assistance with mobility aids, moving safely in and out of bed
  • Housekeeping – cleaning, washing, vacuuming, tidying
  • Appointments and days out – getting to the doctors, hairdressers or going out for the day
